Follow-up/maintenance care for simple or uncomplicated conditions such as acne and other skin conditions, headaches, birth control questions/refills, menstrual or other endocrine conditions. Includes stable depression, anxiety, ADHD, autism, eating disorder (in recovery), general nutrition counseling, etc.

MODERATE: COMPLEX NEW PROBLEM(S) OR FOLLOW UP OF MULTIPLE OR CHRONIC STABLE CONDITIONS

Moderately complex new or previously diagnosed problems requiring more management consideration. Includes mild to moderate medical conditions requiring updated assessment and guidance and mild to moderate mental health concerns, including active symptoms of depression/anxiety, ADHD, eating disorders, substance abuse.

COMPLEX: MULTIPLE COMPLEX NEW PROBLEMS OR FOLLOW UP OF CHRONIC UNSTABLE CONDITIONS

More complex or complicated new or previously diagnosed problems requiring extensive management consideration and care coordination, e.g. significant medical or developmental conditions with active symptoms, worsening depression/anxiety, ADHD, eating disorders, substance abuse.
