December 28, 2022

I would like to thank you for your trust and confidence in me while caring for you and your family members. I regret to inform you that effective Jan 28th 2023, I will be leaving my position in adolescent medicine and resuming my position in pediatric urgent care for family reasons. Thus I will no longer be seeing patients at Girls to Women/Young Men’s Health and Wellness, 4200 S. Hulen St Suite 450, Fort Worth, TX 76109.

I have truly enjoyed my role and the privilege of providing much needed compassionate care to you and your families. It really has been a pleasure to be your doctor. I leave you in the great hands of my GTW/ YM colleagues Dr. Carolyn Lentzsch-Parcells, Dr. Sarah Stone, Dr. Adele Reeder and NP Erin McGuire. I am confident they will continue to provide you with the highest level of care and support. All medical records will continue to be stored at Girls to Women/Young Men’s Health and Wellness. If you wish to continue your care at Girls to Women/Young Men’s Health and Wellness, simply notify the front desk when scheduling your next appointment.

If you choose to seek medical care from another doctor or office, I would recommend doing so as soon as possible. You can contact your local medical society or health care insurer for a referral. If needed, a medical records release form is available on the Girls to Women/Young Men’s Health and Wellness website: www.gtw-health.com. Upon receipt of this signed medical records release, Girls to Women/Young Men’s Health and Wellness will forward a copy of your medical records to you or to the physician you designate.

If you have any questions, please do not hesitate to contact Sharlene Harden, Practice Manager at 972-733-6565.

It has truly been an honor to be your doctor. I wish you all the very best and appreciate all the support and kindness you have given me over the years.

Sincerely,

Naima Garrett, MD
