Kevin Windisch, MD

It is with great sadness and many tears that I inform you that I have made the decision to leave Girls to Women/Young Men’s Health and Wellness effective April 3, 2025. As of that date, I will no longer see patients at Girls to Women/Young Men’s Health and Wellness, 16980 Dallas Parkway, Suite 204, Dallas 75248.
It has been a great pleasure and a privilege to provide care for you over the years. I want to thank you each of you for allowing me to be your doctor. The relationships we have built are deeply meaningful to me.  You have touched my life both personally and professionally, and I have learned so much from each of you. This was not an easy decision, and I will sincerely miss my patients and the GTW/YM staff.

In my place, I recommend you continue your care with the exceptional medical providers of Girls to Women/Young Men’s Health and Wellness. I am confident they will provide you with the highest level of care and support. All medical records will continue to be stored at Girls to Women/Young Men’s Health and Wellness. Simply notify the front desk at GTW/YM that you wish to continue care here when scheduling your next appointment and you will be transitioned to our other providers seamlessly.

If you choose to seek medical care from another doctor or office, I recommend doing so as soon as possible. You can contact your local medical society or health care insurer for a referral. If needed, a medical records release form is available on the Girls to Women/Young Men’s Health and Wellness website: www.gtw-health.com. Upon receipt of this signed medical records release, Girls to Women/Young Men’s Health and Wellness will forward a copy of your medical records to you or to the physician you designate.

If you have any questions, please do not hesitate to contact Sharlene Harden, Practice Manager, at 972-733-6565.

It has truly been an honor to be your doctor. I wish you all the very best and appreciate all the support and kindness you have given me over the years.

Sincerely,

Kevin Windisch, MD
