Cancellation Policy: Appointment no-shows and last minute cancellations disrupt the practice and limit our ability to give patients on our waiting list an opportunity to be treated at an earlier date. We are now requiring a minimum of 24 hours notice (not including weekends and holidays), for cancellation of appointments. Cancellation for Monday appointments must be received by 12:00 PM (noon) the previous Friday otherwise a cancellation fee will be charged.

Cancellation fees are not covered by insurance. The fee is $250 for new patients and $60 for established patients. Two missed appointments by an established patient are grounds for dismissal from the practice. ___________________ (Initial of guardian/patient)
