WELL-CHILD/ADULT CARE (Wellness/Preventive Care/Annual Check-ups)
The physician will review general psychosocial screening information, perform a full physical examination, address immunization needs, and determine if any screening laboratory evaluations are warranted. When appropriate, this may include external genital inspection or internal pelvic examination (with or without Pap testing) as indicated. This visit fulfills the "annual preventive care" benefit for insurance purposes and is typically only covered once per year under most insurance plans. For more information, check with your health insurance carrier about your preventive health care benefits.
PROBLEM-FOCUSED EVALUATION/MANAGEMENT (SICK/PROBLEM VISITS)
Ongoing Primary Care Services
In addition to annual wellness exams, GTW physicians can assume ongoing primary care responsibilities for general health concerns, including acute illness visits and management of intermittent or chronic medical or psychosocial problems. This included services for sick visits (such as strep throat or UTI), medical gynecology (such as vaginitis or irregular menstrual bleeding), and assessment/treatment of complex conditions such as eating disorders, obesity, ADHD, and mild mental health concerns like anxiety or depression.
Consultation Services
In cooperation with a patient's primary care physician, GTW physicians can provide consultation for specific problems (e.g. questions about puberty, menstrual problems, hormone management of acne, management of simple depression/anxiety, etc.). Please let us know at the time of your appointment if you would like us to send correspondence about your care to your primary physician.
Cancellation Policy
Appointment no-shows and last minute cancellations disrupt the practice and limit our ability to give patients on our waiting list an opportunity to be treated at an earlier date. We are now requiring a minimum of 24 hours notice (not including weekends and holidays), for cancellation of appointments. Cancellation for Monday appointments must be received by 12:00 PM (noon) the previous Friday otherwise a cancellation fee will be charged.
Cancellation fees are not covered by insurance. The fee is $250 for new patients and $60 for established patients. Two missed appointments by an established patient are grounds for dismissal from the practice.
Thank you for your understanding and compliance with this policy.
