iLEAP process

 

Before Your Appointment:  Our office will mail or email a series of comprehensive questionnaires. This will include medical and psychosocial history, risk behavior screening, nutrition and dietary behavior screening, and parent surveys around family eating behaviors and attitudes.  These forms are lengthy and provide information CRITICAL in developing an appropriate treatment plan.  We request that you COMPLETE AND RETURN THESE FORMS AT LEAST 24 HOURS PRIOR TO YOUR APPOINTMENT or the appointment may have to be rescheduled. 

 

 

RECOMMENDED READING:  We request that parents (and patients, as appropriate) read or listen to the audio program Intuitive Eating (Tribole and Resch).  These are available for purchase through our office or can be obtained from bookstores or online.

 

 

 

MEDICAL ASSESSMENT AND RECOMMENDATIONS

 

First Appointment (Initial Assessment):  Patients will meet with our physicians for a full medical and psychosocial assessment, including in-depth discussion around food behaviors and body image. Families will be asked to complete any additional screening evaluations as needed at that time.  If not already completed, we will request fasting laboratory studies as indicated.    Labs can be ordered in advance by arrangement with GTW staff. Patients will be instructed where to have labs drawn at an independent drawing station (LabCorp or Quest).  Patients may choose to have their physical examination by their own physician by arrangement. Second Appointment (Review Findings and Establish Treatment Plan):  Patients will meet with their GTW physician to review lab results and discuss findings.  Patients and families will review treatment options and help design an appropriate treatment plan with the physician, drawing on resources such as nutrition education, dietitian services, psychological counseling and support, and fitness training.  Individual, family, and group appointments will be considered.

 

iLEAP TREATMENT / INTERVENTION PLAN

 

CURRICULUM AND/OR COUNSELING APPOINTMENTS (1 MEETING EVERY 1-2 WEEKS)

Research suggests that a minimum of 8-12 weekly sessions are necessary for any significant long-term behavior change to take place.  GTW physicians and staff will guide each family through a process of appointments with specialists from multiple disciplines, including nurses, health educators, counselors, dietitians, and fitness specialists according to individual patient and family needs.  Participation in visits varies according to specialist, chosen curriculum content, and family need.  For example, parents may meet with a dietitian to discuss family feeding dynamics, a patient may bring a sibling to a counseling appointment to discuss teasing about weight at home, or an entire family may come to a fitness appointment to learn a home exercise program.  iLEAP providers are expected to communicate regularly with each other to maximize mutual understanding of each family’s needs.

A GTW program coordinator will work with patients to facilitate this process and answer questions or address concerns on an ongoing basis.

 

PHYSICIAN APPOINTMENTS (APPROXIMATELY ONCE PER MONTH)

Patients and parents (as applicable) will meet with their GTW physician approximately once monthly to review progress and monitor for clinical changes (including blood pressure, weight, and other physical parameters as indicated).  This meeting presents an opportunity to evaluate and modify the treatment plan as necessary, allowing families and providers to work together to shift the focus of their program to accommodate new concerns.
