·         Physician Visits.  Physician services will be billed to the family or the designated insurance plan as appropriate.  These are typically considered “problem-focused” medical appointments, not annual “physical examinations” or well-child care.  They may be considered a “covered service,” depending on the resulting diagnoses and in accordance with the contract between you and your insurance company.  It is the patient’s responsibility to know their terms of coverage and to make any necessary arrangements to pay for these services. If applicable, your co-pay or other arranged payment obligation is required as usual. 

 

 ·         Non-Physician Services.  As recommended by your physician, appointments may be scheduled with an affiliated Registered Dietitian, Psychosocial Therapist, Fitness Specialist, or Health Educator. 

o   These services are not typically covered by your insurance.

o   We encourage you to call your insurance company to inquire about or verify coverage for these services.  (In our experience, plans are more responsive to member inquiries than to physician offices.)

o   You will receive appropriately coded receipts for services to file with your insurance as desired. 

o   Co-pay is not required in addition to these fees.

o   Fees are paid to the participating provider at the time of service.

Cancelled or missed appointments:  GTW requires 24-business hours notice to avoid full charge for the cancelled or missed appointment.
